PCN Roles

General Practitioner (Recently Qualified) 2 sessions per week

Our recently qualified GP rotates between our practices, assisting them by providing clinics containing additional GP appointments.

Diabetes Specialist Nurse

The Diabetes Specialist Nurse provides clinical advice, specialist skills, knowledge and expertise to health care professionals and people with diabetes and their family/carers.

Clinical Pharmacist 4-5 sessions per week

Clinical Pharmacists can manage a number of health conditions within their competencies, e.g. long term condition reviews and medication reviews. They can deal with any drug supply issues and medicines management within the PCN.

Clinical Pharmacist appointments are also available at weekends at certain practices within the PCN.

Pharmacy Technician

Pharmacy Technicians provide support to the Clinical Pharmacist team, by conducting medicine audits, medicine reconciliation, managing prescriptions, counselling patients on medicines and supporting their structured medication reviews.

Care Coordinator

The care coordinator works as part of a multidisciplinary team (MDT) within the practices to identify people in need of proactive support. A key part of the role is in the care homes MDT: improving the continuity of care by acting as a point of contact for residents, families and professionals.

First Contact Physiotherapist 2-3 sessions per week

Physiotherapists are available for first consultations at the GP practice.

Around one in five people book to see their GP when they experience bone, joint or muscle problems, when they could actually be safely and effectively seen by a physiotherapist at the first point of contact.

First Contact Physiotherapists have expertise in the assessment and management of bone, joint and muscle problems. They assess patients, diagnose their condition, give advice on how best to manage the problem and if needed refer for investigations or to other specialist services.

If you have bone, joint and muscle problems, please contact your GP practice and ask for an appointment with one of our First Contact Physiotherapists. Due to the COVID 19 pandemic currently the service is open for telephone and video assessments with the option for face to face assessments if needed.

Health & Wellbeing Coach

Health and Wellbeing Coaching is a service offered to help create lasting change to improve a person’s well-being and overall health. Coaching will provide safe, non-judgemental space giving patients knowledge, skills and confidence to manage their long-term conditions. Your coach will work alongside you to discover what your health and wellbeing goals are.

First Contact Dietitian

Dietitian appointments are now available for first consultations at the GP practices across North Wolverhampton PCN. This service is open for telephone, video or face to face appointments.

Dietitians are qualified and regulated health professionals that assess, diagnose and treat dietary and nutritional problems at an individual and wider public-health level. They use the most up-to-date public health and scientific research on food, health and disease which they translate into practical guidance to enable people to make appropriate lifestyle and food choices.

The aim of this service is to work together with each individual to achieve personalised dietary goals relating to a range of different health conditions; for example, weight management, diabetes, high cholesterol, high blood pressure or irritable bowel syndrome etc. Please contact your GP practice if you think you would like an appointment with the dietitian.

Nursing Associate 2 sessions per week

Nursing Associates enables registered nurses to provide care. Their responsibilities include supporting and supervising Trainee Nursing Associates, performing and recording clinical observations, supporting wound care management, promoting health and wellbeing to patients, advising and signposting patients, and maintaining health records.

Physician Assistant 1 session per week

Physician Assistants manage undiagnosed cases by history taking, physical examinations and clinical decision making to establish a working diagnosis and management plan. Although they currently do not have prescribing rights, they can prepare prescriptions for GPs to sign.

General Practice Assistant 1-2 sessions per week

The General Practice Assistant (GPA) is able to arrange clinical support by completing referrals, tests, follow-ups, simple clinical observations and explaining procedures to patients. They also provide administrative support through letters and completing forms.

Social Prescribing Service

Wolverhampton North Network has launched our own in-house Social Prescribing service available to all patients registered at any of our practices.

Social Prescribing is a non-medical approach to improving your health and wellbeing by putting you in touch with voluntary and community organisations across Wolverhampton.

There is a wide range of areas of assistance that can be offered, including:

  • Debt/benefits advice
  • Walking groups
  • Gym classes
  • Friendship groups
  • Arts and crafts
  • Training and volunteering opportunities
  • Aids and adaptions issues
  • Social isolation
  • Welfare rights
  • Bereavement counselling
  • Counselling

Social prescribing is not designed to replace medical support but getting involved with groups and activities for things that you are interested in can lead to better mental and physical health. It aims to:

  • help improve the quality of life of patients through education and low level support
  • enable patients to develop friendships and networks
  • raise patients’ awareness of available services

Your GP teams or community nursing teams are amongst the people who can refer you to the Social Prescribing service.

If you are referred you will be contacted by our Social Prescribing Link Worker, who will contact you and explain the service in more detail to you. By discussing your individual needs, we can find the best way to offer support, in a personalised and confidential manner.

Pharmacists

The Clinical Pharmacist deals with medication reviews and supports the practice with clinical audits to improve delivery of service. They are able to see patients for acute and chronic conditions and prescribe. The clinical pharmacists team include Oti GPHC 2074238, Mandeep GPHC 2229694 and PCN Clinical pharmacists

Advanced Care Practitioners

Advanced Clinical Practitioners (ACP) like Anne- Marie-Doran are health care professionals that have studied at Masters Level or demonstrated the equivalent through a rigorous process that allows them to practice at an Advanced Level. They have almost all the same practicing privileges as a Doctor and are similar to Specialist Nurses based in hospitals. They could be from a background of Nursing, pharmacology. Our ACP are prescribers and can support with triage, home visits, treat acute minor illness and chronic disease management. They will ensure clinical policies and procedures are updated and implemented, leads and support for the Partners and Practice team to deliver quality service.

Nurses

Practice Nurses have specialist interests in chronic disease management and minor injuries; they also do cervical smear tests, family planning, childhood and other vaccines, ear syringing, diet and lifestyle advice, 24 hour ambulatory blood pressure monitoring, removal of dressings/stitches, travel health advice. HCA's & Phlebotomists

Healthcare Assistants can do new patient health checks, phlebotomy, smoking cessation, B12 injections, wound care management, plus other treatment room tasks. We have our own trained phlebotomy staff, who will take blood samples following request from clinician.

Business Manager

Registered General Nurse 1992 RGN, Masters Business Administration Sheffield Hallam University 1997 (MBA), Visiting Lecturer for Leadership and Management teaches health care professionals at degree and masters level. Mrs Bagary is the Partner Business Manager; she has been part of the practice since it began in 2005. She deals with overall Strategic and Operational needs of the practice and development. Provides robust support and assistance to staff and organisations, main focus for all to provide and deliver a quality service for patients.

Assistant Manager Joanne is part of the senior management team and will deal with operational needs of the practice, to ensure all are adhering to practice policies and procedures and supporting practice with compliance requirements..

Data Administrators and coders

To support practice with compliance and data entry.

Team Leaders/Senior Reception

Team Leaders and Senior Receptionist support the senior management team to ensure all are following practice policies and procedures to deliver a quality service.

Reception and Admin

Administration and Reception Staff are available to all patients either for making or discussing any issues or concerns and giving general advice during surgery hours. All staff are polite, courteous and helpful throughout your visit, show respect and dignity to all service users and professionals. In return we ask all service users and professionals to co-operate with the staff, to help us provide the best service possible. Health Visitors

Tel: 01902 444028 A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
